full details of the matter and the relevant judicial officer which is the subject of your complaint;
gathered any attachments or documents that support your complaint;
clearly expressed your complaint in terms of why you are dissatisfied and how this has impacted you.

You can complete this form online and submit it to the Judicial Council or if you prefer, the complaint form (PDF, 141.7 KB) is also available for you to print and fill in by hand.
